Session 1: Understanding, Goals, and Planning

The first session focuses on understanding the participant as a whole.

We explore:

  • Individual goals and priorities

  • Current medications and how they are used

  • Daily routines and support environments

  • Areas of confusion, concern, or risk

This session forms the foundation for a personalised medication support approach that aligns with real life needs rather than idealised plans.

Session 2: Medication Education and Understanding

This session focuses on improving understanding and confidence with medications.

We support participants to understand:

  • What their medications are for

  • When and how they should be taken safely

  • How medications fit into their daily routine

  • Safe use of PRN medications

  • When to seek advice and who to contact

Information is explained in clear, practical language and reinforces advice already provided by pharmacists and prescribers.

Session 3: Building Routines and Medication Safety

This session focuses on turning understanding into consistent, safe practice.

We work collaboratively to:

  • Build simple and flexible medication routines

  • Reduce missed doses and reliance on memory alone

  • Set up practical strategies and reminders

  • Support safe medication storage and handling

  • Improve confidence recognising side effects or concerns

The goal is to create routines and safety practices that remain consistent over time, even when support needs or staff change.

Session 4: Consolidation, Confidence, and Independence

The final session focuses on reinforcing progress and supporting independence.

We:

  • Review key learning from previous sessions

  • Strengthen routines and safety strategies

  • Address remaining questions or concerns

  • Support participants to feel confident managing medications moving forward

This session helps consolidate gains and reduce reliance on ongoing support where possible.

Medication Support Vs
Medication Management

Medication management and medication support are different but complementary.

Medication Management
A clinical role provided by doctors, nurses, and pharmacists. It involves prescribing, reviewing, and adjusting medications.

Medication Support
A non-clinical role focused on helping participants understand their medications, use them safely in daily life, build routines, and know when to seek advice. It does not involve changing medications or making clinical decisions.

Both are important. Medication management ensures the right medicines are prescribed, while medication support ensures they are used safely and consistently in everyday life.

Who Can Provide Medication Support Under NDIS

Medication support under the NDIS can be provided by trained support workers and specialist services working within a non-clinical scope.

It includes education, prompting, routine building, safety awareness, and communication support. It does not include prescribing, reviewing, or changing medications.

Clinical decisions and medication management remain the responsibility of doctors, nurses, and pharmacists. Medication support services work alongside these professionals to reinforce understanding, support safe use, and keep participants and care teams aligned.

Clear role boundaries are essential for safe and effective care.
