PharmaClear provides NDIS medication support services to help participants use their medications safely, confidently, and with greater independence.

PharmaClear delivers medication support in a non clinical and non administrative way. Our services are designed to complement, not replace, pharmacists, GPs, nurses, or clinical medication management.

Our support is delivered through a structured four-session model, with flexibility for ongoing sessions where needed, and is participant-led, reinforcing understanding and confidence rather than giving instructions.

The first session focuses on building a clear picture of the participant and their medication needs.
During this session, we:
Explore individual goals and priorities
Discuss current medications and how they are used day to day
Identify challenges, confusion, or safety concerns
Understand daily routines and support environments
This session helps identify medication related risks and forms the foundation for a personalised medication support approach that aligns with the participant’s real life, not ideal scenarios.

This session focuses on improving understanding and confidence with medications.
We support participants to understand:
What their medications are for
When and how they should be taken safely
How medications fit into their daily routine
Safe use of PRN medications
When to seek advice and who to contact
Information is explained in clear, practical language and reinforces advice already provided by pharmacists and prescribers. The aim is to reduce confusion and help participants feel informed and in control.

We work collaboratively to:
Build simple and flexible medication routines
Reduce missed doses and reliance on memory alone
Set up practical strategies and reminders
Support safe medication storage and handling
Improve confidence recognising side effects or concerns
The goal is to create routines and safety practices that remain consistent over time, even when support needs or support workers change.

During this session, we:
Review what has been learned across previous sessions
Strengthen routines and safety strategies
Address any remaining questions or concerns
Support the participant to feel confident managing medications moving forward
This session helps consolidate gains and reduce reliance on ongoing support where possible.

Who we are
PharmaClear is a team of registered pharmacists with strong clinical backgrounds who have chosen to work in a non-clinical way to support people with disability.
Through our day-to-day work in healthcare, we have seen the gap between medications being prescribed and medications being safely used in real life. We saw how easily understanding, routines, and confidence can fall through the cracks, especially when care is complex and support teams change.
PharmaClear exists to work in that gap.
We use our clinical knowledge to provide practical medication education, safety support, and routine building, without replacing doctors, nurses, or traditional pharmacy services. Our focus is on ongoing support that builds understanding, confidence, and independence for NDIS participants and the people who support them.

We don’t take over clinical administration like nurses do.
However we assist with safe routines and teach participants and care teams how to manage meds confidently. If we notice a clinical concern, we escalate it to the right health professional quickly. -

We’re based in Melbourne and offer home visits, community-based support, and virtual sessions where appropriate.
