Our Services

What We Cover in Our First Session

We learn about your goals, your current medication challenges, and how your daily routine and support needs affect how you take your medicines — so we can tailor a plan that builds safety, confidence, and independence.

Objectives:
A clear picture of medication risks
A plan tailored to goals and daily life
A strong foundation for measurable progress over time.

Tailored Medication Education

We explain what your medications do, when and how to take them safely, and how they fit into your daily routine. We also build confidence with PRN medicines and help you recognise when to seek support — so you feel safe and in control.

Objectives
Greater confidence and understanding of medications
Safer decisions with PRNs and changes
Clearer communication with healthcare providers.

Building Sustainable Routines

We help you create simple, flexible medication routines that fit your daily life — with reminders and strategies that reduce missed doses and make medications easier to manage every day.

Objectives:
More consistent medication routines with fewer missed doses
Building independence that continues even when support or staff changes.

Safety, Confidence and Independence with Medications

We help you recognise side effects, know when to ask for help, and set up safe medication storage at home — building confidence so managing medications feels clear, not stressful.

Objectives:
Safer medication use with reduced risk of mix-ups and incidents
Better recognition of side effects and when to seek help
Increased confidence and independence managing medications long-term

Who We Support & How It’s Funded

We work with self-managed and plan-managed NDIS participants who want to feel more confident and independent with their medication.

Because our support focuses on building skills, independence and safe medication habits, our services are funded under Capacity Building - particularly Improved Daily Living and Health & Wellbeing outcomes.

We make sure medication isn’t only just “taken on time” but it is well understood by our client, owned, and integrated into their everyday life.

PharmaClear

PharmaClear

PharmaClear is a medication support service created by registered pharmacists to help NDIS participants feel safer, more confident, and more independent with their medications.

We’ve seen firsthand how complicated medication routines and sudden changes such as post hospital discharge, can leave people without the education or support they need.

Our Company Ethos

Our mission is simple:
To build clarity, reduce risk, and make daily life easier.

We take the time to understand each person’s goals and challenges, then provide personalised education and routine support that actually fits their real life. No judgement. No confusion. No medical jargon. Just practical guidance, delivered with patience, empathy, and clinical expertise.

With strong collaboration across support teams we help prevent avoidable harm and support participants to make confident, informed decisions about their health.

Medication shouldn’t be a barrier to independence.
We’re here to make sure it becomes a pathway to it.
