What Is NDIS Medication Support?

NDIS medication support helps participants use their medications safely in everyday life while building skills for independence.

It is a non-clinical service that focuses on:

• Understanding medications
• Building safe and consistent routines
• Developing skills for daily living and independence
• Improving medication safety and awareness

Medication support does not involve prescribing or changing medications. It works alongside doctors, nurses, and pharmacists to support safe use and reinforce understanding.

Our Service

Our support is delivered through a structured four session model, with flexibility for ongoing sessions where needed. Sessions are participant led and focused on reinforcing understanding and confidence rather than giving instructions.

Who we are

PharmaClear is a team of registered pharmacists with strong clinical backgrounds, working in a non clinical way to support people with disability.

Through our experience in healthcare, we have seen the gap between medications being prescribed and safely used in everyday life. We saw how easily understanding, routines, and confidence can fall through the cracks, especially when care is complex and support teams change.

PharmaClear exists to work in that gap.

We use our clinical knowledge to provide practical medication education, safety support, and routine building, without replacing doctors, nurses, or traditional pharmacy services. Our focus is on ongoing support that builds understanding, confidence, and independence for NDIS participants and the people around them.

  • Our support is funded through Capacity Building budgets because we’re focused on building skills and independence with medications. These areas include:

    • Improved Daily Living

    • Health & Wellbeing

    We can help you (or your Support Coordinator) work out the best spot to claim it.

  • We don’t take over clinical administration like nurses do.
    However we assist with safe routines and teach participants and care teams how to manage meds confidently. If we notice a clinical concern, we escalate it to the right health professional quickly.

  • 100%. We provide documentation showing the skills being built, goals worked toward, and the improvements made over time. We use various tools to measure the progress of our clients and the effectiveness of our services.

  • Just simply contact us! We’ll chat through your needs, check NDIS funding, and set up a plan that fits your goals. Our aim is to make it as least stressful for everyone involved.

  • We’re based in Melbourne and offer home visits, community-based support, and virtual sessions where appropriate.

  • 100%. As registered pharmacists and NDIS providers, we follow strict confidentiality, privacy and record-keeping requirements. We only share information with people you’ve consented to (like your GP or support team), and only when it helps keep you safe.
